RMAT Application Form
The candidates who wish to appear for the test need to buy the application form. The form and RMAT information bulletin can be obtained from the office of the coordinator at Udaipur, by sending a Demand Draft for Rs. 1,000 in favor of the ‘Coordinator RMAT’, payable at Udaipur.
- The deposited amount for RMAT is not refundable. It is also not transferable or adjustable for any future exam.
- The appeal that the candidate did not receive the information for appearing in the RMAT exam will not be accepted to refund the deposited amount or any other dues.
- Candidates can also obtain the application forms from various centers as given below. They can also choose one out of these institutions as their choice for appearing the test.

Submission of the Application From
RMAT application form is to be filled properly by candidate’s own handwriting. The form shall be submitted along with relevant documents before the last date of submission. Candidates can post the application form either by post or by dropping in the drop box. However, application forms sent by courier will not be accepted in any circumstance. Candidates can send their forms in either of the way, as given below.
- Candidates may drop the application form in the drop box placed at the RMAT office. Application forms should not be given out to any person in any case. Institution/agency or any other institute is not authorized to collect the form on behalf of RMAT.
